Phoenix Tumbling & Gymnastics Ltd – Waiver and Release of Liability


    Please review and sign this waiver for all participants. You must be the participant, their parent/legal guardian, or have permission from the parent(s) to act as a “Responsible Adult” and sign on their behalf.

    This is a legally binding document. Do not sign unless you agree to its terms.

    By signing, I acknowledge and agree that:
    1. I am 18 years or older and voluntarily participating in the gymnastics, tumbling, trampolining, free-styling etc. (“ACTIVITIES”) at Phoenix Tumbling & Gymnastics Ltd.
    2. If signing for a minor, I confirm I am their parent/guardian or have parental permission to act as a Responsible Adult.
    3. I understand the ACTIVITIES involve risks, including serious injury, disability, or death, which cannot be entirely eliminated despite safety measures.
    4. I accept full responsibility for my conduct and any resulting injuries, medical expenses, or property damage.
    5. I will follow all posted rules, instructions, and safety guidelines provided by Phoenix Tumbling & Gymnastics.
    6. I confirm I have no medical conditions that would make participation unsafe.
    7. I waive, release, and discharge Phoenix Tumbling & Gymnastics from any liability for injury, illness, death, or property damage arising from participation.
    8. I consent to medical treatment if needed and agree that this waiver remains valid for future visits unless revoked in writing.
    9. I understand that personal information will be stored for accounting and insurance purposes.
    10. I acknowledge that Phoenix Tumbling & Gymnastics is not responsible if I or the participant appear in another customer’s photos or videos shared online.

    I confirm that I have read and agree to this waiver, Phoenix Tumbling & Gymnastics Ltd’s Privacy Policy, and understand the inherent risks involved. I sign this document willingly.
    I confirm I am both fit to take part in gymnastics and have read and understood this form / I am filling this in on behalf of an under 18 whom is fit to take part in gymnastics:
